Feline "Fatty Liver" Hepatic Lipidosis


Dr. Dr. Gordon: My cat is very sick. I took him to the vet because he had become very lethargic. After the vet ran some tests, he determined that my cat has “fatty liver” disease. My cat is very overweight to begin with, but I did not know that he could get this disease just by being “fat”. The vet said that treatment can be very difficult. Can you help me understand this disease? RK

Dear RK: I am very sorry to hear that your cat is not feeling well. From time to time, veterinarians encounter cats that develop fatty liver disease or hepatic lipidosis. This is a disease that is specific to the cat, since its’ dietary requirements and metabolism are much different than the dog’s metabolism. Let me explain.

If a cat, especially an overweight cat, stops eating, it can suffer serious liver problems. Cats are extremely dependent upon having protein in their diets. Unlike many species, cats cannot adequately synthesize their own protein from other building blocks. If a cat is not taking in enough protein, it will start breaking down proteins from its body, including important muscle and liver proteins. At the same time, the body is starving for energy, so it mobilizes fat stores for calories, and carries this fat to the liver for processing. Normally, the liver would metabolize these fats to use them for energy, store them, or secrete them through bile. If the liver is missing some of the proteins needed for fat metabolism, all a liver cell can do is store the fat. The fat builds up inside the cells, a condition called hepatic lipidosis, or "fatty liver,"and the cells swell, pinching off bile ducts. Bile, with all its waste products, cannot empty from the liver into the intestine as it normally does, and the cat becomes jaundiced.These changes in the liver occur within 36 hours of not eating. Since the liver cannot metabolize the fat for energy, the cat suffers further starvation, setting up a vicious cycle of fat mobilization and liver damage. Although this buildup of fat inside liver cells is often associated with diseases that cause decreased appetite, or metabolic disorders such as diabetes, we also see cases that are idiopathic, or have no identifiable underlying cause.

The diagnosis is made by doing blood tests that may first indicate an increase in the liver enzyme values. Usually an ultrasound exam of the abdomen should be the next step, which usually shows the liver that is extremely “shiny” on the ultrasound image. A fine needle aspirate can then be sent to the lab so that the pathologist can definitively diagnose the problem.

Treatment of the condition can be difficult. If there are any underlying diseases that are causing the cat to have stopped eating (infection, inflammation, etc.), these must be addressed to allow the cat to feel better. But the cornerstone of therapy is to get the cat to eat once again. Many times, this is more difficult than it sounds, for the cats in this situation usually do not feel well enough to have any appetite, and they need a lot of calories to offset the metabolic effects of the disease. To counteract this, the veterinarian will usually suggest syringe feeding a very high protein and high carbohydrate diet up to 4 times a day. Sometimes appetite stimulants are used to encourage eating. If the patient is resistant to syringe feedings, a special feeding tube may need to be surgically placed in order to provide the cat with adequate nutrients to reverse the process. Once the cat starts eating on its own, the feeding tube can be removed.

Fortunately, most cases of fatty liver are reversible with intense veterinary care. Although most cats present being very ill and lethargic, the majority are able to recover as long as they begin eating on their own. This may take several days of hospitalization at the veterinary clinic however. Cats with hepatic lipidosis will then require dietary adjustments to help the cat return to a normal or near-normal weight. This should always be supervised by a veterinary professional. In addition, supplements such as Liver Terrain (liver support) , Oxicell, Probiotic and digestive enzymes to assist with fat digestion e.g. Lypozyme can be helpful as well. Diet For Cats With Digestive Problems, IBD and Intestinal Lymphoma

Q: My 16 year old Tonkanese cat has trouble with digestion. He sometimes has small hard stools or diahrrea and/or vomiting. He primarily likes sea food. The Vet sold us Royal Canin for digestion health. I'm not so sure about the ingrediants. Do you think this is a good choice. (cat doesn't like it) any other suggestions for a food that would help him? Vet says it could develop into IBD. Thanks for your help

A: Thank you so much for submitting a question. Your kitty's digestive problems are very common for senior cats. It seems that a lot of cats as they get older develop digestive problems which can include diarrhea, constipation, vomitting and discomfort. Many pet owners try the prescribed "prescription diet" from their veterinarians and the kitty doesn't want to eat it. Moreover, the prescription diets often contain ingredients such as corn, byproducts and other synthetic ingredients that may not be good for your cat.

What is hard for cat owners to understand is why their cat "suddenly" developed these issues. Most likely, the symptoms of diarrhea, poor digestion, vomitting, etc were evolving over time and now as a senior cat, it has become more apparent. What this means is that the general "inflammation" in the intestinal lining has been building and as your veterinarian mentioned, if untreated over time, can lead to intestinal lymphoma in cats. There are medications that can help but if possible and the symptoms are not too severe, starting with some supplements and a diet change is the best step.

The main issue is that you need to avoid poultry, sometimes seafood and most grains. You don't want to use a cat food that contains wheat gluten for example. All of these ingredients are hard for cats to digest and can be big allergens for cats. I am reluctant to recommend a specific brand as many of the high quality diets are very high in protein and depending upon your kitty's liver or kidney values, I would want to make some adjustments to the diet. In other words, if I recommended that you use Instinct rabbit by itself (which can work great for many cats), that may not be advisable for your kitty if the kidney values are even slightly elevated, which they often are in a 16 year old cat. What would be best is to schedule a telephone consultation with a veterinary professional so that whoever is making the dietary recommendation can review your cat's laboratory results along with the evaluation of the current symptoms. Certainly adding some canned pumpkin to whatever diet you use will help add valuable fiber and helps regulate bowel motility.

Dietary changes alone, at this stage, will not be sufficient. It is very important that you also incorporate some natural nutrients into your cat's diet. Golden Retriever is Scratching and Itching Constantly

Q: Dear Ariel: My 5 year old Golden Retriever Ginger is scratching, chewing and itching constantly. I can't sleep because she wakes me up shaking the bed. She licks at her private parts and has some small red sores on her tummy. I am feeding her a good dog food Nutrimax and bring her to the veterinarian but he just keeps giving her more antibiotics and shots. Is there anything else I can do?

A: So sorry to hear that Ginger is scratching and itching. This can be a big problem especially during this time of year. In Memory Of Beloved Robert Weinstein, My Stepfather


Our dear Bob passed away yesterday. He just celebrated his 83rd birthday on June 5th. He made an incredible fight but the pneumonia and complications from open heart surgery were just too much for him.


Bob was a thoughtful, generous and caring person who took care of Selma, our mother for 38 years. They had a wonderful life together and enjoyed terrific times going on cruises, shopping and just being together. He could never do enough for Mom who was disabled with multiple sclerosis, diabetes and then later Alzheimers. His only concern was always what was best for Selma.


Bob was a wonderful father and stepfather and gave so much of himself. He was so kind to others in his community too and for years volunteered his time to bring in baked goods for the seniors in his apartment complex.


Bob suffered terribly as a result of the complications due to his surgery. We are so grateful he is at peace now and will miss him terribly. We love you Bob. You will live forever in our hearts!

You want to do everything you can to save Dusty and never realized how important his diet and weight were to his health. You think back on all those delicious treats you gave him and now worry that your loving care may have somehow harmed him. Dog Scratching, Dog Constantly Itching, Dog Hot Spots-- Holistic Care Can Help!!

Most people think that when their pet is scratching and itching, it must be fleas. But most concerned pet owners are routinely using preventative flea products and yet their pets continue to scratch and itch. If a pet owner is using flea preventative products and the itching still continues, then further investigation is warranted.

The hot weather seems to be the worst of times for our poor pets. They chew, lick, scratch, scoot and tear their hair out. Their paws turn red and they get ear infections as well as hot spots. While it could be fleas, mites, parasites or mange, most of the time, it is allergies! Just as many of us experience hay fever and develop respiratory symptoms, our pets with allergies often develop skin issues. Allergies can be both environmental and food related.

Conventional veterinary treatment for allergies involves treatment with cortisone, antibiotics, allergy shots, antihistamines, shampoos and topical treatments. All of these can be very helpful for our pets and give them much needed immediate relief. But many of our pets have much more chronic problems and the symptoms return as soon as the medicines wear off. This is where holistic veterinary care can become especially valuable. Holistic veterinary care involves the use of a hypoallergenic diet and supplements to address the pet’s underlying condition. Many times pets have a chronic yeast condition which weakens their immune system. Pets with allergy problems typically have digestive disorders and since a great deal of our immune system is located in the GI tract, there is a direct correlation. Minimizing allergy symptoms is often directly related to quieting the inflammation in the GI tract.

Here are some general points about allergies you should be aware of:

1) Allergies are cumulative. In other words, a pet may have both food and environmental allergies but the symptoms only become noticeable during certain times of the year. This doesn't mean your pet doesn't have allergic reactions all the time. It just means that the situation builds up similar to a balloon that fills up with air until it pops. In your pet’s case, it may mean that all of a sudden your pet has hot spots or starts chewing on its paws or rear end during the hot summer months but most likely has underlying digestive issues all year round.

2) Cats and dogs may display allergy symptoms differently. Cats may have respiratory symptoms, vomiting and other digestive disorders due to allergies as well as skin reactions. Dogs may have scratching, itching, chewing on fur, ear infections, scooting on their rear end and weepy eyes.

3) Most of the time, pets have BOTH food and environmental allergies. There is no one medication or supplement that will cure your pet’s symptoms. Skin problems are tough and require a comprehensive effort. Using the following protocol for your pet can be helpful:

Topical treatment in the form of therapeutic baths (available from your veterinarian) and natural sprays (If your pet is allergic to grass, try washing paws after going outside).

Special hypoallergenic diet—Keep the following ingredients out of your pet’s diet. There is no “one” allergy diet that works for everyone. Each pet is different. 4) Use supplements to help your pet cope with allergy season to reduce inflammation and overall discomfort. Be sure to include a generous amount of Omega 3s from fish and flax oil, bioflavanoids such as quercetin, digestive enzymes and an effective probiotic. The Environmental Protection Agency (EPA) has increased restrictions and cautioned consumers to use extra care when applying topical flea treatments. We understand how important it is to use flea and tick prevention, and wish to remind you that many diseases arising from NOT using flea and tick preventatives can cause serious, even deadly consequences. We, are not advising that all clients should stop using them, rather, that caution should be taken and that each pet's specific situation should be evaluated. What we are recommending is that you review the EPA advisory below and schedule an appointment with us to discuss the best options for your pet. Of special concern are cats, certain breeds of dogs and any immune-compromised or sick pets. Here is the press release issued by the Environmental Protection Agency:EPA to Increase Restrictions on Flea and Tick Products Cautions Consumers to Use Products with Extra Care Release date: 03/17/2010 WASHINGTON – Due to a significant increase in adverse incidents, the U.S. Environmental Protection Agency is taking a series of actions to increase the safety of spot-on pesticide products for flea and tick control for cats and dogs. Immediately, EPA will begin reviewing labels to determine which ones need stronger and clearer labeling statements. Next, EPA will develop more stringent testing and evaluation requirements for both existing and new products. EPA expects these steps will help prevent adverse reactions. In dogs and cats that can include skin effects, such as irritation, redness, or gastrointestinal problems that include vomiting or diarrhea, or effects to the nervous system, such as trembling, appearing depressed or seizures—from pet spot-on products. “EPA is committed to better protecting the health and safety of pets and families in all communities across our nation,” said Steve Owens, assistant administrator of EPA’s Office of Prevention, Pesticides and Toxic Substances. “New restrictions will be placed on these products, and pet owners need to carefully read and follow all labeling before exposing your pet to a pesticide.” Following the 2008 increase in incident reports, EPA received additional information from the pet spot-on pesticide registrants and others and began an intensive evaluation of these products. Today, EPA is reporting the results of this evaluation, and taking steps to address the spike in reported incidents. Among immediate actions that EPA will pursue are: · Requiring manufacturers of spot-on pesticide products to improve labeling, making instructions clearer to prevent product misuse. · Requiring more precise label instructions to ensure proper dosage per pet weight. · Requiring clear markings to differentiate between dog and cat products, and disallowing similar brand names for dog and cat products. Similar names may have led to misuse. · Requiring additional changes for specific products, as needed, based on product-specific evaluations. · When new products are registered, granting only conditional, time-limited registrations to allow for post-marketing product surveillance. If there are incidents of concern associated with the product, EPA will take appropriate regulatory action. · Restricting the use of certain inert ingredients that EPA finds may contribute to the incidents. · Launching a consumer information campaign to explain new label directions and to help users avoid making medication errors. In addition, to improve the regulatory oversight of pet products, EPA will require more standardized post-market surveillance reporting on adverse effects, require submission of more sales information so the agency can better evaluate incident rates, and bring up-to-date the scientific data requirements on pre- and post-market testing so they are more in line with the Food and Drug Administration’s requirements. Flea and tick products can be appropriate treatments for protecting pets and public health because fleas and ticks can transmit disease to animals and humans. While most people use the products with no harm to their pets, the agency's analysis determined that smaller dogs tend to be disproportionately affected by some products and that the exposure of cats to some dog products is a concern. People should carefully follow label directions and monitor their pets for any signs of an adverse reaction after application, particularly when using these products for the first time. EPA recommends that owners consult a veterinarian about the best way to protect their pets from fleas and ticks or whether pesticides are needed, especially before using any product on weak, aged, medicated, sick, pregnant or nursing pets, or on pets that have previously shown signs of sensitivity to pesticide products. EPA is coordinating these actions with Health Canada as Canada also identified similar concerns about the use of spot-on flea and tick products last year, and with the Food and Drug Administration’s Center for Veterinary Medicine. The agency is inviting public comment on how best to implement these new measures. A Federal Register notice announcing the opening of a docket will be published on March 19, 2010.
